Medical Social Worker (MSW) - Home Care
|
|
Interviews, coordinates, and plans programs and activities to meet the non-medical needs of patients and their families. Coordinates and plans with outside agencies and home care nursing staff for post-discharge care. Requires a master's degree in social work and 2-4 years of experience in the field or in a related area. Familiar with standard concepts, practices, and procedures within a particular field. Relies on experience and judgment to plan and accomplish goals. Performs a variety of tasks. Works under general supervision; typically reports to a senior case manager. A certain degree of creativity and latitude is required.
