Case Manager (Non-RN)

Requirements and Responsibilities
Coordinates the overall interdisciplinary plan of care for a patient, from admission to discharge. Acts as a liaison between patient/family and healthcare personnel to ensure necessary care is provided promptly and effectively. Typically requires certification or licensure in a specialty area with but is not a registered nurse. Typically reports to a head of a department/unit. Typically requires 2 to 4 years of related experience. Gaining exposure to some of the complex tasks within the job function. Occasionally directed in several aspects of the work.
